vn2.7Tiroiditis subaguda con ausencia de captación cintigráfica unilateral inicial
Introducción La tiroiditis subaguda, también llamada tiroiditis subaguda granulomatosa o tiroiditis de De Quervain, es una condición inflamatoria transitoria de la tiroides, considerada la causa más común de tiroiditis dolorosa. El cuadro clínico frecuentemente es precedido por una infección de vía respiratoria alta y se caracteriza por dolor de intensidad variable en la cara anterior del cuello asociado a fiebre y manifestaciones tirotóxicas habitualmente leves. Ambos lóbulos tiroideos están comprometidos desde el inicio en la mayoría de los pacientes y en el cintigrama tiroideo no se visualiza captación del radiofármaco. Presentamos un caso clínico de tiroiditis subaguda de evolución atípica que comenzó en un lóbulo tiroideo y luego se extendió al contralateral lo cual fue objetivado en el cintigrama tiroideo. Caso clínico Mujer de 45 años portadora de un rasgo talasémico. Consultó por un cuadro de 3 semanas de astenia, taquicardia y sensación de calor, agregándose en la última semana dolor cervical anterior lateralizado a izquierda, de intensidad leve y fiebre de 37,8 °C. Tenía el antecedente de una virosis respiratoria alta dos semanas antes. Al examen físico destacó el aumento de volumen exclusivo del lóbulo tiroideo izquierdo, de 30 g, firme e intensamente sensible; el lóbulo derecho era palpable pero indoloro. Se sospechó una tiroiditis subaguda y se solicitaron exámenes (Tabla 1).La paciente estaba hipertiroidea y el cintigrama tiroideo mostró ausencia de captación del lóbulo izquierdo (Figura 1). La VHS fue 29 mm/h, tiroglobulina sérica 113 ng/mL (VN: 2,7-21 ng/mL) y los anticuerpos anti-tiroglobulina, anti-tiroperoxidasa y anti-receptor de TSH fueron negativos. Después de un mes de permanecer hipertiroidea y con febrículas diarias, inició aumento de volumen y dolor en el lóbulo tiroideo derecho asociado a gran compromiso del estado general. Un nuevo cintigrama mostró ausencia total de captación en la región tiroidea. Se inició tratamiento con prednisona 0,5 mg/kg/día el que se mantuvo por 3 semanas desapareciendo rápidamente el dolor, la fiebre y el compromiso del estado general. A los 4 meses de iniciados los síntomas estos habían desaparecido y la función tiroid……
